填申请表有问题 又来麻烦大家

现在天杜伊斯堡的申请表 有几个问题    请各位帮个忙
1  Address 大项里第一栏是c/o 是转交的意思吗？  比如填成 "Max to Martin"  行么？
2 Curriculum Vitae 大项里  有一问是 Which school leaving certificate did you obtain? 这指的是休业证明书（用词霸查的 :chijing: ）？抑或是在读证明呢？  我是在读生
2'还是在 Curriculum Vitae大项中 有一小项也叫Curriculum Vitae  下面写的是 Higher education/occupational training from school leaving certificate until today(date of this application)     Please enter your educational bachground/career in chronological order.(Duplicate entries- in addition to the aboe - are desired).  是让把上过的学都再列一遍吗？
3 在Other Applicaitons大项里 有一问 Please name all other German higher education insititutions(colleges/universities) you have applied/intend to apply to:  我总觉得不胜不写 深受中国高招的影响 :bizui:   必须要写么？

还有一点 忘说了
1st choice degree course 里面 有三栏:Major subject， 2nd major or 1st minor subject 和2nd minor subject     
2nd choice degree course里面 也有同样的三栏
不知道是什么意思   除了一个最想去的  其他几个提供的专业我也都想去   该怎么个填法呢
