Information and keys to Registration Form
Dear Mr/Ms,
this information leaflet, which includes the necessary keys, is intended to make it easier for you to complete the Registration Form.
Please read through this information carefully before you complete the form.
This application replaces the letter of acceptance in the notice of admission. It should be submitted by post or in person by the date stated in the notice, along with:
• proof of health insurance policy (Please request the forms at your health insurance company and tell them that it is for the University.)
• if applicable, letter of termination from your university or other institution of higher education (transfer students only and in case this has not already been submitted)
Important information and explanations for the completion of the Registration Form
• Please complete in block letters using the left margin
• Please use capital and lower-case letters
• Please do not fill in the grey areas
• Please refer to the attached keys when completing corresponding questions
• In a number of questions, you are asked to complete information about countries and districts. Please write the appropriate international or national country codes. For towns located within a city’s postal district, please add the letter D (district). Examples are given.
• Should there not be enough available space, please use common abbreviations.
So that we may contact you at any time during your studies, please ensure that the Student’s Office at the FHW-Berlin receives any change of address. This should be submitted in writing (forms can be obtained from the Registrar’s Office), and can, in exceptional cases, be done by telephone or in person.

Thursday 1:00 p.m. to 3:00 p.m.
During semester holiday period:
Monday 1:00 p.m. to 3:00 p.m.
Thursday 1:00 p.m. to 3:00 p.m.
