最近在填学校的网上申请表，有些问题不太明白，麻烦哪位高手帮忙解答一下，十分感谢！

Please enter what type of higher education entrance qualification you hold and when and where you have obtained it.

这个higher education entrance qualification 是指高中毕业证还是指高考，如果指的是高考的话也没有证书啊，难道用高考成绩单？

1. Type ：Entrance qualification for college of higher education
               
                General qualification for university entrance
              
                 Subject-specific qualification for university entrance
                有三个选项，如果是普通的高考的话，应该选哪个？

2. Name of certificate (original name)

3. Date of certificate 高中毕业正上面有时间，但是高考成绩单上好像没有时间啊

4. Average grade

5. Name of school这是指高中吗？
